Notice to Quit Accompanying Form

As of April 1, 2023, Landlords who issue a Notice to Quit for Non-Payment of Rent to a residential tenant must also include an “Accompanying Form” prepared by the Executive Office of Housing and Economic Development (EOHED).

To initiate eviction proceedings, Landlords must have proof that they provided Tenants with adequate notice and a copy of the “Accompanying Form.”
