Maintenance records in MA

There is a form here called a maintenance record, but it looks like it is specific to my repairs while made under one tenant. From what I understand, I need a work / repair log that goes back 3 years, regardless of tenant. This supposedly is so I cannot accuse tenant #3 of damage that tenant #1 caused. Where is that type of work log/form?
