Forum / Move-Out & Eviction / Zero Tolerance for Criminal Activity addendum

My tenant has violated the addendum with both drug/criminal activity on the property. What kind of eviction notice should be served? From my understanding, under these circumstances, the tenant will have 3 days to move out, but the only 3-day eviction form i see available on this site is for non payment of rent.
