And the other co tenant want to get a new roommate? Do I need to create a new lease agreement? or can I just add the new roommate?

I would make sure that you get a release signed from all the tenants releasing the other tenant. And then get a new lease signed by all tenants along with the replacement tenant. Do not forget to screen your new tenant.
