I served a 3 day pay or quit notice from ez landlord today. (California) I noticed it doesn't have my phone number on it. should I be concerned that this will be defective service?

Yes it does, it should have Name, Address and Phone number of the person to whom rent be paid to. I would add it in and resend. I don't see it being a big deal. Just re-do it and send it over again.
