I am willing to have a tenant opt out of a 12-month lease after 3 months by providing a 30 day notice. Landlord forms does not have language relating to this. Does anyone have an idea to how it should be worded?

Make sure that you word it exactly and specifically as you want it to go. For instance....
Landlord and Tenant agree that Tenant may be released from the obligations of the lease provided that the Tenant provides the Landlord with a thirty prior notice. Tenant understands that this option is only available after an initial period of three full months have elaspsed from the beginning of the lease.
