I am leasing directly to a travel nurse agency who is paying for the housing on behalf of the tenant. Do I list both the tenant and the agency? Thank you!

It depends. if the agency wants to rent the apartment and use it as nurses rotate through then lease it to the agency. If it's for a single employee, then lease it to the single employee.
