Ontario Agreement to Terminate Tenancy (Form N11)

This Ontario Agreement to Terminate Tenancy is used when a landlord determines that they want to terminate a lease or rental agreement with a tenant for one of several reasons. If you have a tenant who is persistently late with rent or who no longer qualifies to live in their unit, you may serve them notice to vacate the property by a certain date with this form.

When landlords and tenants mutually agree that tenancy should be terminated, they will use this form to end the lease or rental agreement in question, and the tenant will agree to move out by a stated date on the form.

Who: If you and your tenant agree to end the tenancy, you will serve them with this form, with the appropriate fields filled in, including the tenant’s name and address, your name and address, the address of the rental property, the reason for terminating tenancy, and any and all details leading up to this decision to terminate the tenancy.

What: This Ontario Agreement to Terminate Tenancy is legal documentation that you and your tenant have mutually agreed to end their tenancy. It must not only state why tenancy is being terminated but also when the tenant must vacate the premises.

When: As soon as you and your tenant have agreed to terminate the lease or rental agreement, you may serve them with this notice. Until the deadline for vacating the property has passed, you may not take further action to get the tenant to vacate.
