Who: The owner of a HUD subsidized property and Tenant will both need to complete, sign receive a copy of this completed form (keep a copy for your own records, as well) in addition to a Lease.
What: This is the contract between the HUD landlord and the Tenant and must be attached to the lease.
When: Before move-in.
