Document Description
In Arizona, the landlord has to file a complaint, summons, and civil cover sheet (F.E.D. Packet) with the appropriate court (either Justice or Superior), along with any required fees. Download and prepare this form so that it’s ready to take to the court house.
Who: This packet is completed and given to the clerk of court with any necessary fees.
What: Forcible Entry and Detainer Packet including the complaint, summons, and cover sheet to initiate court proceedings and evict a tenant.
When: This is filed either on the 6th day after the tenant receives the 5 Day Notice to Pay Rent or Quit, or 11 days after the 10 Day Notice to Cure is served to the tenant.
Please Note: Not all forms are accepted in Arizona courts. Please contact the appropriate court clerk in the court of which you will be filing to confirm the use of a particular form, determine whether any additional forms are required, and to verify fees.
