This is the official Landlord/Tenant Complaint used to file for eviction in the Fifth Judicial Circuit; comprised of the islands of Kauai and Niihau.

Please note: There is no fourth judicial circuit. It was eliminated in 1943.

Who: This is filled out and given to the Clerk of Court.

What: Formal Court Complaint to begin eviction court proceedings.

When: This is completed and filed with the district court with any applicable fees.
